    New York State Health Plan Marketplace is the official New York State website. Call the phone number at the top of the page for assistance. You might also find you can complete the application by following directions and providing the information.   
     
    Medical Service/ Dental / Help with the NY Marketplace Enrollment  
    Neighborhood Health Centers has two locations at 155 Lawn Avenue, Buffalo (875-2904) and 3674 Commerce Place, Hamburg, NY 14075 (phone: 648-4345) For assistance with signing up for the NY Health Plan Marketplace, contact this health center. They also offer help with a variety of types of medical and dental care.  
     
    Medical Services Providers that will offer services for low fees or no fee, especially for those without insurance or Medicaid: 
    Check the websites or call for hours and more information.  
    Community Health Center of Buffalo at 34 Benwood Avenue, Buffalo, NY 14214 (phone: 986-9199) is open daily and welcomes walk-ins. They offer medical, dental, social work and family planning at reduced fee rates and sliding fee scales for those without insurance or with high deductibles.   
    Jerico Road Community Health Center has two locations: 184 Barton Street (phone: 881-6191) and 1609 Genesee Street (phone: 892-2775) where they provide health care services.
    Lighthouse Free Medical Clinic at 1609 Genesee Street, Buffalo, NY 14211 provides some services.  
    Harvest House of South Buffalo offers some medical care as part of its ministry to the community. It is also called Good Neighbors Health Care at 175 Jefferson, Buffalo, NY. (phone: 856-2400)
    Planned Parenthood can provide birth control counseling and services, pregnancy counseling and services, sexually-transmitted diseases counseling and services, abortion counseling and services, as well as general health care services at some locations.   
     
    Child Health Plus and Family Health Plus 
    Although the Health Plan Marketplace is changing options for children and adolescents who are under age 19 without health insurance and who are not eligible for Medicaid, these websites may be useful:

    *Some school districts offer plans through a specific company. Call the guidance counselor or school nurse for information. 
     
    For more information regarding health insurance, go to Erie County Social Services and click on "child health plus" or "family health plus." Other topics include "COBRA Continuing Coverage Program" which temporarily might allow an individual or spouse to continue to receive insurance with the group and at the group coverage cost even though employment has ended.
